## Start with a decision, not a model

Healthcare AI covers very different products: documentation assistance, administrative classification, operational forecasting and clinical decision support. Evidence for one product does not establish the value or safety of another. Define the decision the system supports, who remains accountable and what happens when its output is wrong or unavailable.

A useful project brief names the user, the input data, the expected output, the permitted action and the review process. It also describes the existing workflow so the team can compare the proposed tool with a real baseline.

## Separate administrative and clinical uses

| Use case | Evaluation questions | Human control |
| ----------------------------- | ----------------------------------------------------------------------- | ------------------------------------------------------------------ |
| Draft documentation | Are facts supported by the source? What is omitted or misattributed? | An authorized reviewer checks and approves the draft |
| Route administrative requests | Does routing match the service rules? Which cases need escalation? | Staff can correct the category and recover misrouted work |
| Forecast operational demand | Does performance hold across sites, seasons and changing workflows? | Planners review uncertainty and override recommendations |
| Support a clinical decision | What is the intended medical purpose and applicable regulatory pathway? | Qualified clinical governance defines validation and permitted use |

Do not infer a mortality reduction, diagnostic benefit or financial saving from a generic model benchmark. Those are product- and setting-specific claims that need appropriate evidence.

## Establish the data contract

List each source system, identifier, timestamp, terminology and authorization boundary. Decide how late data, missing values and corrections are handled. Separate development fixtures from production data and document retention and access controls.

For EHR integration, [FHIR capability and authorization checks](https://alabenaicha.me/insights/hl7-fhir-integration-guide) are part of the implementation. A model endpoint does not bypass the hospital's access rules. The [clinical workflow guide](https://alabenaicha.me/insights/healthcare-ai-clinical-workflows) discusses integration points; suitability still depends on the specific workflow.

## Build an evaluation set before rollout

Use representative, appropriately authorized examples with a documented reference process. Include normal cases, ambiguous inputs, missing data and failure scenarios. Record both task performance and the time required for human review. A fast draft may not save time if correcting it takes longer than the original task.

For generated text, check unsupported statements, omitted context, identity mix-ups and whether the reviewer can trace statements back to their source. For predictions, examine calibration, relevant subgroups and changes over time. Set acceptance criteria with the responsible domain experts.

## Pilot with a recovery path

Start with a limited, supervised workflow. Log the model and prompt version, the review outcome and operational errors without copying unnecessary sensitive content. Keep a manual route available and define who can disable the feature.

Compare the pilot against the baseline using the same case definitions. Measure completion time, correction effort, escalation rate, availability and running cost. Clinical outcomes require a separate evaluation; operational convenience is not a substitute.

## Review applicability before expansion

Medical purpose, jurisdiction and organizational role determine the applicable requirements. Use the [medical-device software guide](https://alabenaicha.me/insights/eu-mdr-software-medical-device-guide) and [EU AI Act guide](https://alabenaicha.me/insights/eu-ai-act-healthcare-compliance) as starting points for a requirements discussion, then verify current obligations with the accountable specialists.

Scale only when the evidence supports the defined use. Keep reviewing drift, workload and incidents after release. A decision to narrow or stop a use case can be a successful evaluation outcome.
