Epic Integration & Consulting

Expert guidance and development for integrating with Epic EHR using FHIR APIs, HL7v2 interfaces, and the Epic App Orchard ecosystem.

Epic is the most widely deployed EHR system in the United States, and integrating with it requires specialized knowledge of its FHIR R4 APIs, HL7v2 interface specifications, and the App Orchard marketplace process. I provide end-to-end Epic integration consulting, from initial technical assessment and architecture design through development, testing, and go-live support. Whether you are building a SMART on FHIR application, setting up inbound and outbound HL7v2 interfaces, or navigating the Epic App Orchard review process, I support the engineering and validation work required for your approved production integration.

SMART on FHIR Apps Built

What I Deliver

Epic FHIR R4 API Integration

I build applications that leverage Epic FHIR R4 endpoints for patient access, provider-facing workflows, and backend system integration, using the correct OAuth 2.0 flows for each context.

SMART on FHIR App Development

I develop SMART on FHIR applications that launch from within the Epic workflow, enabling clinicians to access your tools without leaving their EHR context or re-authenticating.

HL7v2 Interface Configuration

For use cases where FHIR is not yet supported, I configure and develop HL7v2 interfaces for ADT, ORM, ORU, MDM, and other message types commonly used in Epic environments.

App Orchard Navigation

I guide you through the Epic App Orchard submission and review process, including technical documentation, security questionnaires, and the required testing milestones.

Epic MyChart Integration

I build patient-facing integrations with Epic MyChart, enabling patients to access your application data, schedule appointments, or complete questionnaires through the MyChart portal.

Why Work With Me

Avoid Costly Missteps

Epic integration has specific requirements and approval gates that can catch teams off guard. My experience helps you avoid common pitfalls that delay launches by months.

Accelerate Certification

I know what Epic reviewers look for and can prepare your application and documentation to pass review efficiently, shortening the path from development to production.

Seamless Clinical Workflow

Integrations built to launch within Epic context mean clinicians never have to leave their primary workspace, driving adoption and reducing workflow disruption.

Compliant Data Access

I implement the correct OAuth scopes, consent models, and data handling practices required by Epic and by the healthcare organizations that use it.

Frequently Asked Questions

How do SMART on FHIR launches work with Epic?

Epic supports EHR launch (context and tokens come from the chart) and standalone launch (your app starts the OAuth flow). Both use PKCE; backend services use signed JWTs instead. Sandbox and production differ in scopes and patient data, so plan fixtures early. A practical walkthrough of ISS, aud, launch token, and failure modes is at /insights/smart-on-fhir-app-launch.

What is Epic App Orchard and do I need it?

App Orchard is retired. Listing is now Epic App Market, Showroom, and Connection Hub: Showroom is where customers browse, Connection Hub records connections, and App Market is the current catalogue name. A listing is a go-to-market artefact, not a requirement for a private app at one health system, and it is not a substitute for member-by-member FHIR bases. There is no App Orchard review SLA. SMART launch, sandbox versus production, and backend JWT mapping are covered at /insights/smart-on-fhir-app-launch.

What is the difference between Epic FHIR APIs and HL7v2 interfaces?

Epic supports both modern FHIR REST APIs and traditional HL7v2 message interfaces. FHIR APIs are best for on-demand data retrieval, SMART on FHIR app launches, and patient-access use cases. HL7v2 interfaces are still used heavily for real-time event-driven data feeds like ADT notifications, lab results, and order routing. Many integrations use both depending on the specific data flow requirements.

Can you help with Epic FHIR sandbox testing?

Yes. I help teams set up and use the Epic FHIR sandbox environment for development and testing. I also help navigate the common discrepancies between sandbox behavior and production behavior, which is one of the most frequent sources of integration surprises. I prepare comprehensive test plans that cover the scenarios you will encounter in production.

How do OAuth 2.0 flows work with Epic FHIR APIs?

Epic supports different OAuth flows depending on the use case. Patient-facing apps use the Authorization Code flow with PKCE. Provider-facing SMART on FHIR apps use the EHR Launch flow where Epic initiates the OAuth handshake. Backend system-to-system integrations use the Backend Services flow with signed JWTs. I implement the correct flow for your specific use case and handle token management, refresh logic, and scope negotiation.

What data can I access through Epic FHIR APIs?

Epic FHIR R4 APIs expose a wide range of clinical resources including Patient, Encounter, Condition, Observation, MedicationRequest, AllergyIntolerance, Procedure, DiagnosticReport, DocumentReference, and many more. The specific resources available depend on the API scopes granted by the healthcare organization and the regulatory context (patient access versus provider access). I help you identify which resources you need and which scopes to request.
